COVID-19 Liability Waiver Form

This is a liability waiver form for patients to sign when attending physical therapy during the COVID-19 pandemic.  It is intended to protect the therapist from frivolous lawsuits that claim the therapist/company was responsible for the patient getting COVID.  The form is drafted to be enforceable in states that enforce reasonable liability waivers (some states may not enforce such waivers.)  Your purchase gives you a non-exclusive license to download one copy of the form one within 10 days of your purchase for your personal use.
